Patients who live within our practice area may register with our practice. The area covered by postcodes are as follows; SE14, SE8, SE3, SE4, & SE13.
You should come along to reception with your medical card and fill in the registration form. If you do not have a medical card, we can still accept you with a proof of address. If you are unable to register with our practice, details of other primary Medical Services can be obtained from PALS.
If you would like to see a particular doctor, please inform the reception staff and we will do our best to accommodate you.
New Patient Health Checks
When you register as a new patient, you will be offered a New Patient Health Check. This is to ensure we have an up-to-date record of your medical history, medication and allergies and to ensure patients have had screening tests and immunisations they may be due. A general check will be completed, including taking your height, weight, blood pressure and testing a urine sample. You may also be offered health advice and informed about services offered.
If you are a parent of a child aged five or under and you are bringing them for a health check, please bring their red book or immunisation information with you.
Please note, you need to have a New Patient Health Check before you receive any services at our surgery.
